Factors and Treatment Methods for Cellulite

Cellulite is caused due to distorted fats in the adipose and connective tissues. Exact reason for cellulite formation is not revealed yet. People believed that it is caused due to weight loss and sexual dimorphic traits. Cellulite is believed to involve in changes in the structure of the adipocyte and the surrounding septa, which are more vertically oriented, and allows movement of the adipocyte up to the dermis. Various studies were conducted to study the receptors that are involved in cellulite formation. Some of the receptors that are found in cellulite formation include; peroxisome proliferator activated receptors, uncoupling protein 1, and α-androgen receptors and β-androgen receptors. By analyzing these factors, scientists believed to find easy and long term result to cure cellulite.

The current trend followed in cellulite treatment involves heating adipocytes, remodeling it, and cause dermal remodeling in the septa surrounding the adipocyte cells. Bipolar radiofrequency power and infrared energy with vacuum and mechanical massage method is used in VelaShape for circumferential reduction of cellulite. TriActive, a FDA approved device combines a low-energy diode laser with suction massage to treat cellulite. Similarly Accent, also a FDA approved device uses dual radiofrequency technology for circumferential reduction. The unipolar setting is more effective than bipolar because it penetrates deep dermal and subdermal layers and treats large volume of tissues. SmoothShapes, another FDA recommended tool is also useful in treating cellulite. It combines a laser and an LED light source with mechanical rollers and a vacuum to restructure adipocytes and improve metabolism.

So in the mere future, there are many in and out approaches going to evolve for treating cellulite. Some expected techniques like combining external technologies with internal laser lipolysis to heat adipocytes and eat up septa which provides long term results. Mesotherapy is one method where drugs are injected to eliminate fat and improve metabolic activities in the adipocytes. Some of the mesotherapeutic ingredients are aminophylline, isoproterenol, ephedrine, calcium pyruvate, carnitine, and ma huang. Some detergents like phosphatidylcholine and deoxycholate can also be used in treating cellulite. Retinol Anti-Cellulite, a cosmeceutical from RoC, as well as electrolipolysis, carboxytherapy, and cryolipolysis are expected to be the further treatment methods in the mere future.

Cellulite Treatment

Edematous fibrosclerotic panniculopathy, or local lipodystrophy, better known as cellulite, nodularity, or dimpling that occurs in most women, typically in the buttocks and upper thighs, as well as the abdomen.

Etiology

The etiology of cellulite remains unclear, although several theories have been advanced (J. Cosmet. Laser Ther. 2005;7:7-10; Int.J. Cosmet. Sci. 2006;28:191-206;J. Cosmet. Laser Ther. 2004;6:181-5). Factors that have been cited as important in the pathophysiologic process include sex-specific morphologic differences, vascular changes, inflammation, and deterioration in connective tissue septa (J. Cosmet. Laser Ther. 2004;6:181-5; J. Dtsch. Dermatol. Ges. 2006;4:861-70).

One train of thought suggests that cellulite is derived from a disorder of endocrine-metabolic microcirculatory origin, in which changes in subcutaneous adipose tissue and the interstitial matrix manifest in unsightly bumps (Int. J. Cosmet. Sci. 2006;28:191-206).

Causes of Cellulite

Cellulite forms when the skin’s connective tissues are overcome by progressive fat accumulation in specific areas of the body. According to Howard Murad, the actual cause of cellulite occurs internally with the weakening of the skin cells and connective tissue. Cellulite and stretch marks are caused by skin that has deteriorated and dehydrated to the point where buoyant fat cells trapped just below the skin are able to push into the dermis, the middle layer and the skin and show through the surface. Fat cells increase in size and their membranes become distorted and lose their shape. This results in a dimpling appearance and is further impaired by the build up of toxins within the connective tissues. There are several factors that contribute to the causes of cellulite. Some of these factors are age, genetics, body fat percentage, hydration levels, coffee consumption and smoking habits. In the skin, there are tough strands of fibrous tissues that connect the skin to the rest of the body called the “septa.” This septa anchor at certain points, similar to a net. This net of fibrous septa-tissue exists above the layer of fat in the body. However, fat sometimes makes its way through the holes in this net of fibrous tissue, thus plumping the fat outward. This plumping of fat causes the “cottage cheese” resemblance of cellulite. Many times, even if a woman has a very low body fat percentage, if the skin around that area is pinched, some cellulite can still remain visible. However, most women probably care about cellulite when it is visible without pinching. (4) The make up of the strands of fibrous tissue in men and women are different. Also, the fibrous tissue may be tighter in women, which would cause a much more pronounced plumping effect in women in comparison to men.

Modes of Treatment for cellulite

Although no treatment approaches have been deemed entirely successful, given their typically mild and temporary effects, cellulite therapies have included the use of noninvasive devices such as massage machines, radiofrequency systems, and laser and other light instruments; invasive surgery such as liposuction, mesotherapy, and subcision; carboxy therapy; topical therapy; and even oral modalities (J. Drugs Dermatol. 2008;7:341-5; J. Drugs Dermatol. 2007;6:83-4).

Massage appears to be the most effective modality for low-grade cellulite, as it is conducive to enhancing blood and lymphatic circulation and draining waste products. The effects are temporary, however, as they are with even the most effective topical products, which contain caffeine and theophylline and dehydrate the fat cells, temporarily shrinking them. For the highest-grade cellulite lesions, minimally invasive procedures such as subcision can render improvement (Int. J. Dermatol. 2000;39:539-44).
